Teaching Fundamentals Written Reflection

This form is to be submitted to the TATP office upon completion of all requirement for the TF Certificate (6 workshops) and prior to receiving the paper certificate. Please answer the following questions, taking into consideration what you have learned through your participation in the TATP Workshop Series.

If you would like to print or save your reflection for your own records, you will have the opportunity to do so after you have filled in the form and clicked PREVIEW. You can then print or save the page before clicking SUBMIT.

Please note: As of June 2017, certificates will be verified and printed only on the First Friday of every month.
